DETERMINANTES Y RESULTADOS CLÍNICOS DE LA TITULACIÓN DE LOSINHIBIDORES DEL SISTEMA RENINA-ANGIOTENSINA-ALDOSTERONA YBETABLOQUEADORES EN PACIENTES CON INSUFICIENCIA CARDIACA CLÍNICA DEFALLA CARDIACA DEL COMPLEJO HOSPITALARIO DR. ARNULFO ARIAS MADRID.PANAMÁ, FEBRERO 2017 – NOVIEMBRE 2018

Resumen
Introducción: múltiples estudios han demostrado que los inhibidores del sistema renina-angiotensina-aldosterona (ISRAA) (que incluyen los inhibidores de la enzima convertidora de angiotensina [IECA] y los bloqueadores de los receptores de angiotensina [BRA]) y los betabloqueadores (BB), disminuyen el riesgo de mortalidad y hospitalización por insuficiencia cardiaca (IC), en los pacientes con insuficiencia cardiaca con fracción de eyección reducida (ICFER). No obstante, en la práctica clínica, la proporción de pacientes que alcanzan las dosis estudiadas en estos ensayos es pequeña.
Métodos: mediante un estudio prospectivo, identificamos los pacientes con ICFER, ingresados a la Clínica de Falla Cardiaca del Complejo Hospitalario Dr. Arnulfo Arias Madrid entre febrero y agosto de 2017, y se les dio seguimiento hasta noviembre de 2018. Se definió dosis altas como iguales o superiores a 50% y dosis bajas como inferiores a 50%, tanto de ISRAA como de BB.
Resultados: identificamos 54 pacientes; 35.8% alcanzaron dosis altas de ISRAA y 37.7% lograron dosis altas de BB. La tasa de hospitalización por IC en el grupo de dosis altas versus bajas de ISRAA fue de 0% versus 25% (p: 0.024) y de 0% contra 27.3% en el grupo de dosis altas versus bajas de BB (p: 0.010).
Conclusiones: la utilización de dosis altas de ISRAA y de BB reduce el riesgo de hospitalización por IC en los pacientes con ICFER, en comparación con dosis bajas.
